Misao disappears after suddenly learning that she was Pixy Misa and had been fighting Sasami as Pretty Sammy. Sasami eventually finds her at school—where they first met, but she learns that Pixy Misa is still inside Misao and trying to wrestle back control. As her best friend, Sasami is the only one who can save Misao from the evil magic that's inside her heart.

The episode begins with a flashback to when Misao first met Sasami—when Sasami found her feverish while getting dressed for gym class in a locker room. Misao notes that Sasami was the first friend she ever had.

In the present, Misao is running aimlessly across a bridge with the shocking revelation fresh in her mind. She crosses the bridge and stops and kneels down at a tree and admits the truth to herself, that she was the evil magical girl who caused so much trouble. She believes Sasami is angry toward her and that she has nowhere left to go, and then she begins sobbing.

Rumiya flies to a branch of the tree and blames himself for what’s happened. Suddenly, Misao is having chest pains. Rumiya realizes that Misao forcefully undid her transformation earlier and Pixy Misa is trying to take over again, so he quickly returns to Juraihelm in order to get help from Ramia and begs Misao to be a little more patient. Meanwhile, Misao can hear Pixy Misa’s voice inside her head as she tries to prevent her from coming out again.

Ryo-Ohki explains to Sasami how Misao must’ve been influenced by evil magic into becoming Pixy Misa, and Sasami blames herself for getting her involved because she chose to become a magical girl, thus causing Pixy Misa to be created. She angrily throws away her magical baton and tells Ryo-Ohki that she’s going to look for Misao alone.

Meanwhile, in Juraihelm, Tsunami almost goes down to Earth but is warned by the priesthood that she's only allowed to go to Earth once during the deliberations, and that she’ll be stripped of her qualifications to become queen if she goes again. Tsunami realizes that magic won’t save Misao and begs Sasami to save her friend.

Misao’s parents, Kotoe and Shigeki, call Honoka’s CD store and ask if Misao is there, but she isn’t. Kotoe tells Shigeki that Misao was very excited to see him, and asks when he’ll have to leave again. Shigeki says he has to leave Japan tomorrow morning and starts looking in Misao’s room. He finds the composition sheet of the piano song he wrote for her and finds a small note she wrote saying “I want to see dad”.

Rumiya returns to Juraihelm to seek Ramia’s help, only to find his sister completely dejected and unresponsive after failing to become queen.

Sasami is having no luck finding Misao while running through the city at night. At the same time, Misao is standing under a streetlight, telling herself that she can’t go home. Miyuki—Misao's neighbor and former Lovely Madam “Mrs. One”— finds her and asks why she’s out so late. Misao lies about going home soon but tells Miyuki about her situation without naming Sasami, saying that she’s been unknowingly lying to her friend and that she must be angry with her. Miyuki tells Misao that her friend will understand if she apologizes and tells her exactly what she thinks. Miyuki leaves for home, but Misao still believes Sasami won’t forgive her. She continues walking, thinking about all the things that are important to her, including the place where she met Sasami.

She goes to Umi no Hoshi Elementary and enters the locker room where they met, and she finds Sasami sitting inside, as she had expected her to come here. Misao doesn't want to face Sasami and tries running away, but she suddenly collapses from another bout of pain. Ryo-Ohki arrives and explains that she’s trying to change back to Pixy Misa due to the evil magic inside her heart. Sasami asks how she can help Misao change back, and he says that while magic isn’t omnipotent, it will react to people's feelings toward others; thus, Sasami, Misao’s best friend, is the only one who can remove the evil from her heart. Sasami understands and takes hold of her magical baton. Once she transforms into Pretty Sammy, she enters Misao’s heart, which is blackened from evil.

Sammy soon finds Misao inside of an egg-like barrier, but Pixy Misa is also there. Sammy tells Misa to change Misao back and leave, but Misa says that she and Misao are one and the same, as she was created out of the sadness in her heart in addition to her magical powers. Misa tells her that Misao had developed a complex about being timid and envying the "cheery" Sasami, and she accuses Sammy of harboring a sense of superiority and enjoyment from always covering for Misao. Sammy denies it and goes over to Misao, but she's unresponsive inside of the barrier. She tells her that there still so many things she wants to do with her as friends, but Misao doesn’t respond. However, she continues and says that even if she were to remain Pixy Misa forever, she would still love her. This causes Misao to respond, and the barrier separating them shatters as they reaffirm their friendship.

The moment is interrupted as several snake-like bodies representing the evil magic within Misao’s heart attack them. Misao is initially scared and cowers in Sammy’s arms, but Sammy tells her that they’re going to “do it together”. Sammy and Misao then both hold the magical baton and use Pretty Coquettish Bomber to destroy the creatures.

Misao’s heart brightens as the evil is vanquished, and Pixy Misa peacefully disappearances before Misao and Pretty Sammy, with Misao saying goodbye to her “other side”. In the real world, Ginji and Honoka go to Umi no Hoshi Elementary in the morning and find Sasami and Misao sleeping next to each other in the locker room. Rumiya is also there in bird form and briefly watches before flying away, and he silently thanks Pretty Sammy and says goodbye to Misao. At the same time, Shigeki is on an airplane bound for overseas, but he vows that he’ll come back again soon and see his daughter.

Later, Ginji drops Sasami off at Misao’s home while noting that she’s suddenly acting like a grown-up. When Sasami presses the doorbell, she shocked to find Pixy Misa waiting for her! Only, it’s just Misao who had created a wig and costume to cheer herself up. Sasami asks Misao if she’s sad that she didn’t get to see her father, but Misao tells her that she’s fine because she is here with her.

Trivia

  • As with the previous episode, The Magic Fades Away, this episode uses a unique title sequence, being a simple white screen with the text of the episode title.
  • Although Misao Amano is often portrayed as a meek girl, this version differs from the OVAs and novels, where she was chronically ill with anemia and asthma, respectively. However, as a nod to that characterization, the first time she met Sasami, Misao appeared to be ill, showing symptoms such as a fever from an unspecified cause. However, given that Misao is depicted as a very healthy, albeit physically weak, girl in the show, it is likely that the fever was only caused by a temporary condition, such as a flu.
  • This episode is additionally another rare instance of using a different screen for the eyecatch—this one showing Misao instead of Pretty Sammy, and it's also the only one to use different music.
  • In what's most likely an attempt to keep the episode's mood serious, Pretty Sammy's transformation sequence has been shorted and uses different music than the usual theme.
  • Miyuki is the only Lovely Madam to appear after they are defeated in the episode Farewell Team Lovely Madams!.
  • Among the English-language titles, this episode's title is the only one that's just a single word.
